Do you accept this requirement The Population Health Data Strategist at NC Medicaid's QPHE team facilitates a transformative project, enhancing data exchange between payors and providers for quality improvement. Responsibilities include, but are not limited to: Provide subject matter expertise on QPHEs project to leverage NC HealthConnex for quality and population health, including project design, the implementation of healthcare provider supports, and developing strategies for future phases of work. Lead related strategic efforts related to the collection and exchange of data to support care management programs and programs that address social drivers of health Serve as liaison between QPHEs Population Health staff, Healthy Opportunities Pilot staff, and the core staff leading the NC HealthConnex project Ensure technology and business partners have formative information to plan and adapt their work in adherence to best practices, current policies, and stakeholder demands SKILLS Project Management skills: Adapt to shifting priorities and needs of multiple stakeholders Critical thinking and problem solving skills Communication skills: Ability to communicate complex material in a clear and concise manner both verbally and in writing Understanding of data policy, healthcare data exchange, health information technology, population health and care management programs and SDOH Relationship building: Demonstrated skill in creating and maintaining collaborative working relationships with internal and external stakeholders Microsoft Office products: Strong proficiency in Excel, Outlook, Power Point and word Medicaid: Familiarity with Medicaid and health care associated rules, regulations, and standards Design: Knowledge and experience in design of population health delivery models, care delivery models, care management, clinical program innovations Workflows and processes: Understanding of clinical workflows, including workflows of care management teams Keywords: North Carolina Date Analyst- Mid Level (733556)-DHHS-NC-Need Local Candidate [email protected] |
